I am registered with the Adam’s Practice, Heath Cottage, High Street, Lytchett Matravers, Poole, Dorset BH16. On Tuesday 13th October I went to the Reception desk of the above to make a doctors appointment, was advised that I would need to fill in a form, which I did on the day. I have persistent pain in my right groin/hip area ongoing for many years shown on my medical record. I have been under my Consultant Mr K R Gallagher at Bournemouth Hospital for nearly 2 years and, now on the Hip Replacement list for nearly 6 months.
